What is an EOB?

An EOB is an explanation of how your plan benefits were applied to your claims, so in other words what your insurance has paid and has not paid. Your EOB's will explain any amounts you still owe, what amounts have been paid and anything you need to action (such as completing a claim form).

How are medical claims processed?
Medical Claims in the USA
  1. Seek treatment from a network doctor
  2. Hand over your insurance ID card at the time of treatment
  3. Claims will be sent directly to the claims team for processing
  4. We recommend you complete a claim form for each new injury/illness
  5. Very Important! Track the status of the claim online, or by calling the number on your ID card about 30 days after your visit to the doctor.
  6. If further information is needed, such as medical records, this is your responsibility to obtain and submit to the claims team.

Medical Claims outside the USA,
or Non-Network Doctors
  1. Seek treatment from any provider you wish.
  2. Pay for their services directly and keep copies of all medical records, invoices, bills and receipts.
  3. Complete a claim form for each new injury/illness and submit it along with the above documents to the claims team for processing.
  4. Very Important! Track the status of the claim online, or by calling the number on your ID card about 30 days after your visit to the doctor.
If further information is needed, this is your responsibility to obtain and submit to the claims team.
How do you get reimbursed for medications?

If you have been given a prescription medication by the doctor, please do the following:

  1. Visit a local pharmacy that accepts the Magellan Rx Network and hand over the prescription from your doctor.
  2. You will not be required to pay up front and the claims information will be sent directly from the pharmacy to the insurance company for processing.
  3. If you are required to pay up front for any prescription medication, complete a claim form and include your original receipts and the Rx label of the medication to be reimbursed for your eligible expenses.
